The intake form takes about 15 minutes to complete and includes your personal information, insurance details, and questions regarding your child's development, needs, and health history.
Before you begin…
Make sure you have the following handy:
Your driver's license
Your child's insurance information
Please Note: Prior to your evaluation, we will need a referral from your child's physician. You can request that your doctor fax it to our office at 678-623-5518, or we will be happy to reach out and request it on your behalf.
What happens next?
Once we receive your completed form, a member of our team will reach out within 2-3 business days to schedule your evaluation. At your evaluation, you and your provider will discuss a personalized therapy plan for your child.
