When you refer a patient to us, you can trust that they will receive individualized, family-centered care from a dedicated occupational therapist who genuinely cares about making a difference.

How to Refer a Patient
Pediatricians can complete the online referral form or fax the referral directly to our office at 678-623-5518.
Once the family has completed their intake form, we will reach out to their child's physician to request the referral on their behalf.
